Care Plan Library Overview & Navigation Guide

This is a narration/script for a video about the Care Plan Library on careplans.com and instructions for how to navigate the system.

References & Currency

Each issue statement may be accompanied by a reference in APA format.

To keep the system as current as possible, a computerized process periodically scans for references older than 5 years and searches for more current evidence when found.

Structure of Each Care Plan Issue

Outcomes

• Each issue may have one or multiple suggested outcome statements
• Each outcome includes a suggested number of days until evaluation:
  • = 90 days ? considered short-term goal/outcome
  • = 365 days ? considered long-term goal/outcome
• The system calculates days from today’s date and displays a suggested evaluation date
• Each outcome includes:
  • Description
  • Rationale
  • Optional APA-formatted references (also periodically reviewed for currency)

Each issue includes one or more associated actions, each containing:

• Description
• Rationale
• Optional APA-formatted references

How to Access the Care Plan Library

• From the homepage ? select “Care plan library” in the left side menu
• Or directly enter: https://careplans.com/careplans in the browser URL bar

Library Main Page Features

• Initial view: issues listed in alphabetical order
• Search input field + dropdown list of clinical categories

Available Clinical Categories (as of video creation)

• Mental Health
• Infectious Disease Care
• Cardiovascular Health
• Respiratory Health
• Endocrine and Metabolic Health
• Gastrointestinal Health
• Sensory Perception
• Cancer Care
• Neurological Health
• Musculoskeletal Health
• Urological and Reproductive Health

• Functional and Mobility Health

Filtering & Searching

• Select a category ? click/tap Submit
• To clear category: hover over dropdown ? click the “x” ? Submit again
• Search: enter term/phrase ? Submit
  • When category + search term are both used ? searches only within that category
  • Searches issue title, description, and rationale
• Reset button: clears inputs and redraws page

Right of Submit/Reset buttons: link “Don’t see what you’re looking for? Ask and we’ll help!” ? redirects to contact form routed to a clinical editor.

Pagination & Selection

• Shows number of issues found + number of screen pages
• Page icons clickable to jump to specific pages
• Scroll to see list of issues ? click/tap desired issue to view detail screen

Non-logged-in users: only first four words of content shown + prompt to login or join.

Care Plan Detail Screen (Logged-in View)

1. Issue statement

3. Record ID, code, number of times selected
4. Reference (APA format) – if available

   • Description

   • Suggested days until evaluation + projected date

Left Menu Panel Options (Detail Screen)

Three checkboxes (all selected by default):

• Show Details – uncheck to hide all outcome/action content except labels
• Show References – uncheck to hide all APA references
• Select All – uncheck to deselect everything (allows picking specific outcomes/actions)

Export Options

Below checkboxes: export selected content in multiple formats

• PDF (Acrobat)
• XLSX (Excel)
• DOCX (Word)
• RTF (Rich Text)

Options include: landscape/portrait layout or narrative format.

Downloaded care plans can be individualized, saved, or uploaded into another EHR system.
